Case-specific questions about policy and procedures should be directed to BPD.
Questions can be sent to BPD by regular mail, e-mail, or fax. BPD forwards questions about Medicaid eligibility to HFS's BMEP for their response. Questions are answered within 8 workdays. A copy of the answer is sent to each person copied on the original question.
Question Sent by Regular Mail or Fax
For help in deciding how a policy affects a specific case, complete a Request for Specific Case Guidance (Form 2150). Complete all the information on Form 2150 that applies to the case in question (case name and number, the category [or categories], manual reference number, contact person, etc.). If needed, attach any supporting documents. If the question involves a pending appeal, pending application, or pending pre-hearing conference, include the date.
The answer on Form 2150 is for that case only and cannot be used or cited for other cases.
NOTE: For FCRCs, send Form 2150 using procedures set by the Family Community Resource Center Administrator (LOA). Copy the Regional Training Coordinator and LOA on all Forms 2150 sent.

A Form 2150 policy question can be e-mailed. BPD is listed in the Outlook address book as follows: firstname.lastname@example.org.
When e-mailing a case-specific policy question, always include your name and your office name and location. Also include all the information that is on Form 2150 (case name and number, category, manual reference number, application or appeal date, etc.).
If you need to send supporting documents, fax them to BPD at (217) 557-5850, ATTN: Policy Question. Include in your e-mail that you are faxing additional material.
