In order to qualify under the homeless criterion, the FCRC must verify that the person is homeless and the reason why.
- (FCRC) Verify that the person is homeless and:
- (FCRC) Verify the reason for homelessness as follows:
- Evacuation of a building - court order or sheriff's notice.
- Domestic violence - protection order or police report. The client must have been living with the person who inflicted the domestic violence at the time it occurred.
- Fire - newspaper article or Fire Department report.
- Natural disaster - proof that a natural disaster has been declared by either the Governor or the President.
Once homelessness and the reason for homelessness are verified, enter the following codes on Form 552:

Enter code 355 EHO followed by the 6th full month of eligibility. The Initial Prorated Entitlement (IPE) is a month of eligibility if it includes 30 days or more (an IPE of 60 days or more is 2 months of eligibility). |
The case is centrally canceled for the month following the fiscal month entered in Item 80. The client is centrally sent Form 157C and Important Notice Regarding Eligibility for Transitional Assistance Program (Form 3404).
A batch exception message is also sent to the FCRC to convert food stamp benefits to an FS Only case.
Client's Status Changes
- (FCRC) Review the other criteria for being "not employable" when the Family Community Resource Center becomes aware that the client is no longer homeless.
- (FCRC) Once the review is finished, complete Part II of Request for Determination of Employability Status (Form 3372).
Send a copy of Form 3372 to the client as notice of the decision, and file the original in the case record.
- (FCRC) If the client does not meet any of the other criteria, cancel the case using TAR 69.
NOTE: The client must reapply through intake if they do not contact the Family Community Resource Center by the last day of the last fiscal month of eligibility.
Central Cancellation
- (System) Centrally cancels the case when the 6-month period has ended (based on the date in Item 80 code 355 EHO).
- (System) Sends the client Form 157C and Form 3404 to notify them of the action.

- (FCRC) If the client contacts the Family Community Resource Center, take one of the following actions based on the client's situation:
- If the client provides proof that another criterion is met after central cancellation, but before the last day of the last fiscal month of eligibility, restore assistance using TA 12/TAR 98 (see WAG 19-04-02).
Do not require the client to reapply through intake.
NOTE: If the client does not contact the Family Community Resource Center by the last day of the last fiscal month of eligibility, they must reapply through intake.
- If a client claims to be disabled and provides the needed medical information by the last day of their last fiscal month of eligibility, restore assistance once CAU finds the client disabled. See WAG 03-18-01 for the process to determine if the client is disabled. Do not restore benefits until the CAU decision is received.
NOTE: If the client does not provide the required medical information by the last day of the last fiscal month of eligibility, they must reapply through intake.
- Once a decision is made on the client's request, complete Part II of Request for Determination of Employability Status (Form 3372).
Send a copy of Form 3372 to the client as notice of the decision, and file the original in the case record.
Example 1: A person in Group 04 is approved on the regular roll for 12/95. An IPE is issued for 34 days (11/03/95 through 12/06/95). The first month of their 6-month period is 11/95 and the date entered in Item 80 code
355 EHO is 04/96. The case will centrally cancel for 05/96.
If the person contacts the Family Community Resource Center and provides proof that another criterion has been met by 04/26/96 (schedule cut-off), the case remains active.
If the person contacts the Family Community Resource Center and provides proof after 04/26/96 but before 05/06/96 (the last day of the last fiscal month of eligibility), restore assistance.
If the person contacts the Family Community Resource Center after 05/06/96, they must reapply through intake.
Example 2: A person in Group 09 is approved on the regular roll for November. An IPE is issued for 13 days (11/03 through 11/15). The first month of the 6-month period is November and the date entered in Item 80 code
355 EHO is the following April. The case will centrally cancel for May.
If the person contacts the Family Community Resource Center and provides proof that another criterion has been met by 05/06 (schedule cut-off), the case remains active.
If the person contacts the Family Community Resource Center and provides proof after 05/06, but before 05/15 (the last day of the last fiscal month of eligibility), restore assistance.
If the person contacts the Family Community Resource Center after 05/15, they must reapply through intake.
