WAG 02-05-06: Intake Interview
If a person wants to apply by mail, send the Request for Cash - Medical - SNAP benefits (Form 2378B).
If a person who must cooperate in the child support process is determined to be homebound, enter code 02 in Item 63 of Form 552.
Attempt to contact the applicant to complete a face-to-face interview, or to acquire any information needed to complete the determination process.
