Community Service Agreements

  1. Recently I received by certified mail an Amendment to my FY09 Community Services Agreement. What does this mean? Is this placing a funding reserve of 2% on my funding?

    Every provider who has a FY09 Community Services Agreement received an Amendment by certified mail. The Amendment only changes the language in your existing Agreement. It is not implementing a "funding reserve". All Providers needs to sign this Amendment and fax it to (217) 524-5529.
