SST Referral Process Flow Chart (pdf)
- Provider agency or family reaches out to PAS/ISC for assistance with a person with a developmental disability in a medical or behavioral situation that challenges their ability to live and thrive in the community.
- PAS/ISC identifies that a family or provider agency needs assistance with a person with a developmental disability in a medical or behavioral situation that challenges their ability to live and thrive in the community.
- DDD Network or BTS staff identifies that a family or provider agency needs assistance with a person with a developmental disability in a medical or behavioral situation that challenges their ability to live and thrive in the community.
- PAS/ISC possible interventions: do their own review, perhaps holds a staffing, link to other community services such as community mental health or a behavior analyst, collects information for additional staffing request, refers the person to another provider, completes a request for SODC TA or CART.
- If the above possible interventions are not adequate, PAS/ISC reaches out to DD Network or BTS staff for the following possible options:
- schedule a CART
- arrange a SODC TA
- process additional staffing funding request
- refer to SST
Note: CART is NOT required prior to referral to SST
- DDD staff (Network and BTS) collectively decide what additional support will be offered and if a referral to SST is made. The decision is based on a prioritization of individuals' needs and the capacity of the SSTs.
- If SST is chosen, a referral is made to SST by DD Network staff or BTS staff -- AFTER the Provider agency and PAS/ISC are notified that a referral will be made.
