Summary:

  • Form 1229A, Nursing Home/Supportive Living Facility Redetermination Report Form, is revised to include new sections entitled "Income Diversion" and "Voters Registration Information." 
  • The customer's failure to complete the income diversion section will not affect their eligibility for medical benefits.
  • Redeterminations for Nursing Home (NH) and Supportive Living Facility (SLF) customers with a spouse or dependent family members in the community are due annually in the month of March.
  • Maximus, an external vendor and part of the Illinois Medicaid Redetermination Project (IMRP), mails Form  1229A to all NH and SLF customers.

Revisions to HFS 1229A

A new section entitled "Income Diversion" has been added to Form 1229A.  Failure to complete the income diversion section (Section D) of Form 1229A does not affect the customer's eligibility for medical benefits. However, failure to complete this section will affect the amount the customer must pay to the facility.  Do not cancel the customer's case if this section is not completed.

The income diversion allows the customer the opportunity to start, continue, or increase the amount of income diverted to the community spouse or dependent family members.  Completion of this section also documents the amount of income the customer wants to divert. To divert income, the customer must verify the income of the community spouse or dependent family members.

"Voters Registration Information" has also been added to Form 1229A and the Illinois Voter Registration Application (SBE R-19) is included with Form 1229A when mailed to the customer.

Fixed Redetermination Date

An annual redeterminations is completed in March for all NH and SLF customers with a community spouse or dependent family members who live in the community. Maximus mails Form 1229A to these customers in January. This fixed date allows the customer the opportunity to start, continue, or increase diversion of income based on the cost of living adjustment (COLA) received from the Social Security Administration.
