Subject:  Revised Early Intervention Assessment Instruments

This notice is to inform you that effective August 24, 2015, the list of Approved Early Intervention Evaluation and Assessment Instruments has been revised.

This list includes newly approved evaluation and assessment instruments, some name revisions and/or updated editions. At the end of this list are several assessment instruments that will no longer be accepted for use with Early Intervention Evaluations/Assessments effective February 1, 2016. If you are using any of the instruments that are being eliminated, please plan on identifying approved replacements.

All other instruments listed can be used immediately provided you are properly trained and credentialed to use them based on the developmental needs of the child and family.

To be clear, the CFCs will no longer accept instruments from the discontinued section as of February 1, 2016.

Additionally, understand this list may be updated occasionally, so continue to watch for any updates to ensure the instrument of your choice is approved for use for Early Intervention Evaluations/Assessments. The list will be posted on the DHS website at
